Does anyone know where the First Providian Life & Health insurance Co. is located in New York?

  3. I found several pieces of information. 1.The New York State Insurance Department has a detailed search form for NY insurers. First Providian does not come up there (see below for links to the Insurance Department and the Search Applet). 2.However, on their site I did find an address on a complaint listing for First Providian, dated 1998 : 520 Columbia Drive; Johnson City, NY, which is what an earlier answerer noted (link below). 3.Then I did a little more digging and saw, per link below, a .pdf file saying that First Providian had become part of AUSA Life Insurance Company (page 8) on October 1, 1998. AUSA’s location at that time was 4 Manhattanville Road; Purchase, NY 10577. (914) 697-8000 4.AUSA merged with Transamerica Life Insurance Company on April 1, 2003 (link below) to form Transamerica Financial Life Insurance Company. The home office and phone number is listed as the same as AUSA’s. 5.Finally (I think), Transamerica Financial Life Insurance Company is now part of AEGON. They have a number if you are unsure about which company you should call (1-800-625-4212). I’ve included a link to AEGON and to Transamerica Financial Life Insurance Company. They Good luck.
